Chatham-Kent Community Health Centres provide primary care and support services to high priority groups in Chatham-Kent and Walpole Island. The centres offer employment positions in health promotion, social work, and related to programs focused on health, fitness, nutrition, and youth services.

TransForm Shared Services Organization

TransForm SSO was founded by the five hospitals in Erie St. Clair - Bluewater Health, Chatham-Kent Health Alliance, Hôtel-Dieu Grace Healthcare, Erie Shores HealthCare and Windsor Regional Hospital. It is governed by a board of directors of senior executives from each hospital, as well as five independent directors.
