Improved Coordination of Homelessness Services Happening in Chatham-Kent


On September 24, 2019, Employment and Social Services is hosting a forum for homeless-serving partners to explore the benefits of an improved coordinated access system and receive feedback on a draft coordinated access framework. The plan framework, named CK CARES (Chatham-Kent’s Coordinated Assessment, Referral, and Evaluation Services), outlines the process for people experiencing, or at risk of, homelessness to access housing and supports. It is an essential step to smarter, faster, more coordinated housing system.


Coordinated Access, considered a best practice in homeless prevention, is a centralized process for assessing persons in need of homeless services to determine the appropriate service type and housing options. Coordinated Access is designed to streamline access to housing and services.


“The goal of this forum is to get input from our partners on the draft CK CARES framework. To clarify community partner roles within the system and ensure this plan will work for our community” states Chantal Perry, Program Manager, Homelessness Prevention, Employment & Social Services. “Ensuring people have access to the right program or service at the right time leads to better outcomes for them and the community as a whole”.


In addition to the framework review, a new Chatham-Kent Housing Hero award will be introduced to celebrate community success in reducing and preventing homelessness. “Homelessness is a community issue, and we must work together as a community to resolve it. We are excited to have Mayor Canniff present our first Housing Hero award to someone who has done exceptional work to keep at-risk residents of Chatham-Kent housed” states Polly Smith, Director, Employment & Social Services.


The CK CARES forum is being held at Aristo’s Banquet Hall, 307 Grand Ave. W. Chatham (located inside Smitty’s Family Restaurant) from 8:30am to noon on Tuesday, September 24, 2019.
