When your doctor orders certain tests, procedures and appointments at Chatham-Kent Health Alliance, you can now, from the comfort of your home:
• connect to CKHA's Patient Appointment Request Service
• request appointment times that will be scheduled closest to your preferred time
• view a reminder of your appointments
• Please note: If your appointment is of an urgent nature (requires scheduling this week or next) or if your appointment is for a patient under 12 months of age, please contact the Patient Appointment Office directly by phone at 519-437-6012 or toll free at 877-726-2543.

Hours of Availability:
Your electronic request can be sent 24 hours a day. Chatham-Kent Health Alliance's Patient Appointment Office is staffed Monday through Friday, 7:30 am to 4:00 pm except on weekends and statutory holidays.
