You must complete an Application for the Coordinated Access Waiting List administered by Chatham-Kent Housing Services (CKHS). CKHS is located at the Health & Family Services building, 435 Grand Avenue West, 1st floor, Chatham, ON. Applications are also made available in all Chatham-Kent communities at housing provider offices, municipal centres and municipal information desks. Completed applications are used to assess eligibility for subsidized social housing. Those who qualify are entered onto the coordinated access list for the housing providers selected by the applicant.

How Do I Obtain An Application?

Pick up an application form at CKHS or have them mail one to you by calling Chatham-Kent Housing Services (CKHS) at 519-351-8573 during regular office hours: Monday to Friday (excluding holidays) 8:30 a.m. – 4:30 p.m.

OR

Download the application (from the right side link). Clicking this downloadable will open the application in Adobe Acrobat Reader. To complete the application, click on the form fields and type in the proper information. You can then print and mail the completed application, including all documentation and paperwork requested to the CKHS office. You can also, save the application in Acrobat Reader and email it to ckhousingservices@chatham-kent.ca You will still be required to mail and supply CKHS with all documentation requested.

OR

Visit a municipal centre or a municipal information desk, or a social housing provider in your community to pick-up an application form and a CKHS brochure.


Things to Remember When Applying:

Complete the application by following the instructions attached and supply CKHS with all documentation requested. All Applicants 16 yrs or older must sign the application.

When the application is completed (ALL UNSIGNED APPLICATIONS WILL BE RETURNED), CKHS will assess your application for eligibility. Link to the Social Housing Policies & Procedures manual to find out more about eligibility requirements of the coordinated access waiting list.

Applicants are responsible for contacting the CKHS office to update ANY change of information on their application: address, telephone number, family status, etc. within 10 business days of that change. Your application may be cancelled if you fail to provide these changes.
