Public Health, by law, must keep up to date records of shots or immunizations for every student in Chatham- Kent. Each year Public Health Nurses notify families who have a child that is missing information on their record.

 

If you receive a letter or phone call about your child’s record it means that your child may be overdue for a shot or that he/she may have received the shot from the doctor, emergency department or other health care provider and did not call Public Health to update their record. It is important for parents to call Public Health with the name of the shot and the date it was received so that we can update the student record.

 

Public Health is also required to suspend students who do not have complete records or have an exemption form on their file.

 

So, if your child receives a shot from a healthcare provider other than Public Health please call us. Avoid school suspensions.
