When your doctor orders certain tests, procedures and appointments at Chatham-Kent Health Alliance, you can now, from the comfort of your home:
• connect to CKHA's Patient Appointment Request Service
• request appointment times based on your convenience
• view a reminder of your appointments
• receive information to help you prepare for your appointment

Hours of Availability:
Your electronic request can be sent 24 hours a day. Chatham-Kent Health Alliance's Patient Appointment Office is staffed Monday through Friday, 7:30 am to 5:30 pm except on weekends and statutory holidays.
If your appointment is required within 2 weeks, please do not use this electronic request system, but rather call CKHA's Patient Appointment Office.
