It is an internet service that allows patients to request appointment times for their hospital services.

Who should use the Patient Appointment Request Service?
Anyone who requires an appointment at the hospital in Chatham and Wallaceburg, and has an order form completed by their doctor.

Can I book my own appointment using this service?
No. The service is designed to allow patients to request scheduling of appointments within certain date and time blocks. The actual scheduling of the appointment is performed by the Chatham-Kent Health Alliance Patient Appointment Office.

No.

Which hospital services require a scheduled appointment?
The order form that your doctor gives you will point out whether a scheduled appointment is necessary.

Can I request all hospital appointments on-line?
If your appointment needs to be scheduled within two weeks, please telephone the Patient Appointment Office rather than use the on-line service (519 437-6012 or toll-free at 877-726-2542). There are some procedures that must be arranged by your doctor using alternate scheduling methods.

Can my doctor book these appointments for me?
Your doctor may make the decision to book the appointment for you using alternate scheduling methods.

If I need to make an appointment for someone other than myself, what process should I follow?
You will need to create a new user ID and password for each invdividual that requires access to the service.

How do I access the Patient Appointment Request Service?
Go to www.chatham-kent.ca , roll the mouse over the Community Services heading and select Health. The service will be found within the menu choices on the left.

Another option is to type in the address line of your browser: www.chatham-kent.ca/pars.

Can anyone see or access my personal information?
No, unless you share your user id and password with someone else. Only  the hospital staff from Chatham-Kent Health Alliance Patient Appointment Office will have access to your personal health information.


We suggest that you always sign out of the Chatham-Kent Health Alliance Patient Appointment Request Service when you are finished. We also recommend that you close your browser to erase any appointment information in your computer’s cache memory. This will ensure that someone who may use the same computer terminal will not have access to your personal appointment information.

How do I sign into the service?
You must create your unique user ID and password by completing the profile information form. Additionally, to access the Patient Appointment Request Service you must also provide your health card number, your date of birth and your gender.

You must enter the telephone number of a family member or friend who could get in contact with you.

How will I be notified of my scheduled appointment?
You must access the service again, to receive your appointment notification. The appointment is not booked until you confirm.

You can access and view your appointments, at any time prior to your appointment date.

How will I know if my appointment gets changed?
If the hospital has to change your confirmed appointment, the Patient Appointment Office will telephone you.

When is the Patient Appointment Office open?
The office is staffed Monday through Friday, 7:30 am to 5:30 pm except for weekends and statutory holidays.

What do I bring with me to the appointment?
You must bring your doctor’s order form and your health card.

Yes, please arrive 30 minutes before your appointment to register.
